Process Includes:

Sorting, paginating and indexing of records: Records are sorted chronologically within sections, paginated and a full index prepared.

Preparation of a chronology of relevant facts and records analysis: Records are reviewed in detail and a relevant past medical history is summarized then a detailed cross-referenced chronology of events is organized and prepared. The chronology is carefully summarized in simple narrative (glossary for medical terms, abbreviations can also be provided separately).

Identification of discrepancies / missing records: Missing and illegible records are identified and your attention drawn to any discrepancies. Deletion of duplicate records is performed.

Customization: All final reports can be customized to suit unique needs/expectations of our clients.
